Mental Health Blended Case Management

Services

• Blended Case Management assists adults and children with serious mental illness or emotional disorders in gaining access to needed medical, social, educational and other services.
• The goals of the program are to maximize an individual's ability to live independently and productively in the community.

Eligibility

• Individuals with a serious mental health disorder in need of home-based casework services.
• No income restrictions and no cost for service.

Staff

• Staff requirements are a Bachelor's Degree in Social Work, Psychology, Education, Criminal Justice or to be a registered nurse.
