LPN (Licensed Practical Nurse) – ACT

• Seeking Licensed Practical Nurse as part of the ACT (Assertive Community Treatment Team) to provide intensive, recovery-based treatment for the highest risk consumers of mental health resources.
• Assists in managing medication system; documents medication treatment; communicates and coordinates with medical providers; engages in health promotion, education, collaborations with other team members, and assists consumers with understanding medications prescribed and importance of taking medications.
• Link client with appropriate healthcare providers or specialists and follow up with referrals and recommendations

Schedule: Monday-Friday 8-4:30pm; may include nights or weekends dependent on client needs and also on-call component (approximately once every 6 weeks)
