Job Description:
- Seeking Licensed Practical Nurse as part of the ACT (Assertive Community Treatment Team) to provide intensive, recovery-based treatment for the highest risk consumers of mental health resources.
- Assists in managing medication system; documents medication treatment; communicates and coordinates with medical providers; engages in health promotion, education, collaborations with other team members, and assists consumers with understanding medications prescribed and importance of taking medications.
- Link client with appropriate healthcare providers or specialists and follow up with referrals and recommendations
Medical specialties:
- Psychiatry
- Intellectual disability
Schedule: Monday-Friday 8-4:30pm; may include nights or weekends dependent on client needs and also on-call component (approximately once every 6 weeks)

Work Location: In person
$2,500 Sign on Bonus is for qualifying positions. Must work full-time – 40 hours per week. The agreement will be reviewed and discussed upon hire.
