ACT Team Case Manager

**$2,500 Sign on Bonus for qualifying positions. Must work full-time – 40 hours per week. The agreement will be reviewed and disclosed upon hire.**

Job Overview: The ACT Team Case Manager functions as an integral part of the Assertive Community Treatment Team to provide intensive, recovery-based treatment for the highest risk consumers of mental health resources. The ACT Team Case Manager serves as a primary case coordinator in order to ensure each client enrolled in ACT is receiving the appropriate services relevant to their recovery goals.

Responsibilities and Duties:

• Participate in all aspects of client’s treatment from admission through discharge; serve as a primary source of service coordination for all clients enrolled in ACT;
• Work collaboratively as part of the ACT Team in order to provide comprehensive care that meets the needs of client team members;
• Respond to and assess emergency situations and incidents; assure that appropriate actions are taken to protect the health and welfare of clients and their families;
• Understand and be capable of performing the functional duties of other staff ACT Team members and be willing to serve as a substitute for the provision of other specialist services when the designated staff team member is unable to do so;
• Attend daily team meetings, individual supervision, and group supervision as scheduled;

 

Education and Experience:

Candidates must have the following qualifications:

• Bachelor’s Degree with major course work in sociology, social work, psychology, gerontology, anthropology, political science, history, criminal justice, theology, nursing, counseling or education required
• Experience working in a behavioral health setting strongly preferred

Work Schedule:  Monday – Friday; On call
